Being away from the husband is a reason for a woman to ask for divorce from a judge. It does not mean at all she is already divorced!

 

So if what you claim is true as you said, she is still married to her 2^nd husband and her 3^rd marriage is void. The Imam who did the marriage should be notified to make needed corrections.
